1.  Please fill out the Inquiry Form (labeled Transform Your Health Today!) 

2.  Fill out the appropriate packet below:​

   • These are fillable PDF files. download the proper packet and save it to your computer then fill out the packet and save it again, then email the packet to newpatient@rh-md.com. You can mail or drop off the packet also.

   • New Patient Female

   • New Patient Male

   • New Patient Pediatric

3.  Schedule the New Patient Appointment and submit your reservation fee. We require a reservation fee to ‘reserve’ your appointment slot. You must cancel your appointment at least 1 week before being eligible for a refund. Otherwise, the funds may be applied to other office services such as nutritional supplements, B12 injections, and educational events.
