New Patient FAQ:

  • If you were interested in scheduling a free initial consult before establishing at the practice, that can be done by contacting our Patient Care Coordinator (321) 806- 7436 or booking directly on our website rh-md.com!

    If you have questions or would like us to send over our new patient paperwork, please contact our Patient Care Coordinator. For all establishing patients, our patient paperwork is required to be filled out and sent back prior to scheduling your first appointment.

  • At your first appointment you will meet with one of our skilled providers and discuss your health goals, health history, and current health symptoms.

    Your provider will then customize a lab order based on the information that was gathered during your visit, and from your completed patient paperwork.

     After your labs have resulted, you would then be scheduled for your New Patient Lab Review. This next appointment would entail a thorough review of your results, in which the provider would create a tailored care plan for you.

    Based on your health complexity, we would then schedule any follow-up appointments accordingly.

  • At the moment, our office is able to bill Medicare. That would include Medicare patients with supplemental insurances. We are unable to bill any Medicare Advantage plans or Medicare Replacement plans.

    We do offer self-pay pricing based on provider and complexity of each appointment. For our self-pay patients, we are able to assist in the reimbursement process by providing you with a super bill of your office visit fees. A lot of patients are successful in getting reimbursed!

  • We highly encourage our new patients to bring in all relevant medical records prior to their first appointment to help us in filling the gaps that may not have been included in your patient paperwork. You can send us your medical records by mail, email, or fax.

    We can also assist you in a medical records release by providing you with the necessary form to fill out so we can take the work off of your hands and request your records from other providers’ offices!
